Athletic physicals will be given on Thursday, April 12, 2012 immediately after school at McIntosh High School.  The physicians from Resurgens Orthopedics will conduct the physicals.  Students may download the physical form from the website or pick one up from Ms. Dyche in the front office.  We must have the parent complete page 1 of the physical form and sign on the front page.  Parents must entirely complete the Parental Consent Form which is also available from the website or for pick up from Ms. Dyche.  Students should report to the gym lobby with $20 (cash or check made payable to MHS).  Students will receive their ticket on a first-come, first-serve basis.  No incomplete physicals will be accepted.
